Patient Forms

It may take up to one month for forms to be completed. Please ensure you have filled out your portion of the form before submitting it to your Nurse Practitioner. Incomplete forms may result in delays.

Please also note that we are working diligently to connect more residents with primary care. As a result, our clinics have become much busier and call volumes have increased. Our reception team is doing their very best to answer all calls as promptly as possible. Verbal abuse toward staff will not be tolerated under any circumstances.  To help reduce wait times, we offer online appointment booking for our Nurse Practitioners, nurses, and social work follow-up appointments. We encourage you to use this option whenever possible.

Medication Renewals

Before contacting your clinic location, please note that it may take up to 7 days for medication renewals. Notify your pharmacy if you require a refill and they will fax the necessary information to our office. Please note that dosage adjustments or medication changes require an appointment.
