SUMMARY:
The Essex County Nurse Practitioner – Led Clinic (ECNPLC) provides primary health care services to the residents of the Towns of Essex, Amherstburg and surrounding communities. Outreach services will also be provided to the residents of Drouillard Rd. (Ford City) in the city of Windsor. We are expanding our services to the Town of Kingsville.
The Nurse Practitioner (NP) works to the full scope of their practice within an interdisciplinary team. They collaborate with all team members and report to the Executive Director and Administration Manager. The NP will see patients during scheduled and intake appointments in the clinic. The NP will see patients in the community through outreach and home visits during regular and extended hours.
Nurse Practitioners are Registered Nurses in the Extended Class and must uphold the standards outlined in the College of Nurses: Practice Standard for NPs and demonstrate Nurse Practitioner core competencies.
ROLE AND RESPONSIBILITIES:
Health Assessment and Diagnosis
- Performs a comprehensive health or symptom directed assessment to patients across the lifespan.
- Synthesizes data from multiple sources to make a diagnosis within the scope of the practice of a PHC NP.
- Spends adequate time with the patient to determine the presence of existing and potential health problems, with a major focus on related lifestyle, psychological, socio-cultural, and environmental risk factors with may influence the patients’ health status.
- Assesses individuals, families and communities holistically.
- Provides PHC services to patients in all developmental stages.
- Documents appropriately via electronic medical records.
Therapeutics/Management
- Initiate and manage care of patients with acute and chronic diseases or disorders.
- Monitor the ongoing therapy of patients with chronic illness by providing effective pharmacological, complementary or counselling interventions.
- Use nursing strategies arising from the best available evidence and consistently incorporates patient’s perspective in care.
- Will consult and collaborate with the physician appropriately according to the College of Nurses of Ontario and per the Collaborative Physician Contract.
Health Promotion
- Collaborates with community agencies to implement strategies to promote health and prevent disease with individuals, families, and groups.
Community Development, Education, and Advocacy
- Determines the need for and implements health promotion, education, and primary and secondary prevention strategies for individuals, families, and communities.
- Uses community assessment data to identify community needs and resources to develop, implement and evaluate appropriate programming in conjunction with appropriate community agencies.
Other responsibilities
- Collaborates with other health care providers, staff and patients.
- Participates in the development of strategic plans, goals, and objectives of the ECNPLC
- Contributes to the development of program planning, including implementing, coordinating and evaluating assigned projects in consultation with other members of the primary care team.
- Acts as a preceptor for nurse practitioner students and participates in the education of other students.
- Participates in the hiring, orientation and evaluation of primary care staff.
- Ensures that patient care statistics are provided as necessary.
- Review medical directives on an annual basis.
- Performs other duties as assigned.
Qualifications:
- Bachelor of Science in Nursing. Master of Nursing preferred.
- Current RN(EC) registration and member in good standing with the College of Nurses of Ontario.
- Demonstrated ability to work effectively in a team environment.
- Proficiency in the use of computers and various software applications.
- Excellent written and oral communication.
- Excellent organizational skills.
- Member of the Registered Nurses Association of Ontario and /or Nurse Practitioner Association of Ontario is recommended.
- Current proof of professional liability insurance.
- Current BCLS certificate.
- Current police clearance for adults and children.
- Second language would be an asset.

SUMMARY

ROLE
The Registered Practical Nurse (RPN) assists in the delivery of primary care services, community outreach, and education, promoting healthy lifestyle choices for the residents of the community.
The practice of nursing is the promotion of health and the assessment of, the provision of, care for, and the treatment of, health conditions by supportive, preventative, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function.
There are areas of overlap between RN and RPN roles, but there are differences as well. These differences are based on entry-level and ongoing nursing knowledge and competencies. The
autonomy of the RPN is influenced by the complexity of the patient’s condition and environment. As client complexity increases, there is a corresponding increase in the need for the RPN to consult
with an NP and work collaboratively with the healthcare team.
POSITION RESPONSIBILITIES
1. Professional Practice:
- Adheres to the code of Ethics and CNO standards of nursing practice.
- Practices within the established scope of practice.
- Acts as mentor and preceptor for new RPN and RPN nursing students.
- Upholds standards and reports concerns about unsafe, incompetent or unethical behaviour of care.
- Participates in quality improvement projects and assists with analysis of these projects
2. Scope of Practice:
- Provide care as per CNO guidelines
- Being accountable at all times for her/his own decisions and actions.
- Based on the Client Continuum the RPN must be involved in providing care to less complex, more predictable, low-risk negative outcomes (autonomous RPN practice).
- Based on the Environment Continuum, the RPN must be involved in providing care in a more stable environment.
- Consults with another nurse when a situation demands nursing expertise that is beyond their competence.
- Performs clinical tasks by participating in assessing, planning, implementing, and evaluating nursing care under the supervision of a Nurse Practitioner and practices within set organizational directives.
- Carries out nursing procedures and other delegated medical tasks as ordered.
- Providing routine care, observing clients’ health, assisting providers and communicating with clients and their families.
- Develops a plan of intervention with clients.
- Counsels clients on the health promotion strategies that promote mobility, medication use and abuse, skin care, diet, exercise, and smoking cessation.
- Ensures that the Clinic is maintained by ordering medical supplies, maintaining inventory, ensuring that equipment is in good repair, and sterilizing and autoclaving as needed. Ensure resources and materials are maintained.
- Assists with relevant administrative tasks.
- Provides triage to clients, both on the phone and as walk-ins.
- Performs work in the lab, as required.
- Provide outreach to patients as needed.
- Ensures appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussion of problem cases, answering client inquiries and directing them to the appropriate treatment. Contribute to case conferences and make referrals.
- Participates in the development, implementation, monitoring and evaluation of treatment, education, screening programs, workshops, counseling and health promotion programs and services for individuals, families and the community by identifying health needs of clients and various other community groups.
3. Continuing Competence and Development of Expertise:
Maintain and continually enhance their knowledge and skills related to all aspects of their nursing practice and ensure that their practice is evidence-based.
4. Nurse-Client Therapeutic Relationship:
- Use a wide range of effective communication strategies and interpersonal skills to appropriately establish, maintain, re-establish and terminate the nurse-client relationship (e.g., introducing self, addressing the client by the name that the client prefers, providing information to promote client choice and enable him/her to make informed decisions, etc.)
- Ensure that all professional behaviours and actions meet the therapeutic needs of the patients (e.g., always include the client as a partner in care, demonstrating sensitivity and respect for the client’s choices, being available for the client for the duration of assignment, etc.)
- Establish and maintain the limits or boundaries in the therapeutic nurse-client relationship.
- Protect the clients from harm and ensure that abuse is prevented or stopped and reported.
ORGANIZATIONAL RESPONSIBILITIES:
- Work as a member of the team and participate in staff development opportunities.
- Participate in staff, team, Board and committee meetings, and other Clinic activities as required.

- Contributes to the Clinic’s work by participating in activities that seek to strengthen collaborative and interdisciplinary teamwork, such as inter-component committees and working groups and strategic planning.
