Welcome to Our Nurse Practitioner-Led Clinics
Our Nurse Practitioner-Led Clinics in Windsor and Essex County provide primary health care that is integrated within the community through established partnerships. We work with the entire healthcare team to address the needs of each patient. Our focus is on wellness and disease prevention.
Our four clinics are part of a unique model designed to improve access to care for the thousands of individuals and families who do not currently have a primary care provider.
